Homewood Animal Hospital

Due to our trip to France from December 17, 2009 to December 27, 2009, I set-up our dog Maverick to be boarded at our regular vet at Homewood Animal Hospital for the duration of the trip. On Thursday December 17, 2009, upon landing in Detroit, I received a voice mail from our vet stating that Maverick has an ear infection that needed treatment and also some antibiotics/steroids for the new growth he developed on his paw, between his toes. I okayed treatment over the phone and told them to give him whatever was necessary.
On December 28, 2009, Ken called Homewood Animal Hospital to see what time would Maverick be ready for pick-up. They informed him that the total bill for boarding and medical treatment was $560. It was understood between Ken and I prior to our trip that he would pick-up the vet bill since I was paying for the trip, including approximately $1,500 for car rental in France that was charged on Chase Credit Card.
Because of the monet he spent in France ($200 for coats/hat and cash withdrawal of 160 euros) he informed me that since he had to pay for January mortgage, he could not afford paying for Maverick.
On that same day, Chris from Homewood Animal Hospital called me at 11:38am and I paid Maverick's bill over the phone for the total amount due of $560 by charging it to my Chase Credit Card.

On Wednesday, December 16, 2009, the eve of our trip to France, I moved all the documents back in place into my dresser in our bedroom at the Idlewild home.

Sensitive Documents Removal

On December 3, 2009, after an heated argument over life insurance policies, mortgage papers, passports, birth certificates and miscellaneous other sensitives documents, Ken threatened to keep all of the important paperwork in his possession and is concerned about him still being the beneficiary of my $250,000 life insurance policy with State Farm. He even went to the State Farm office to verify that I did not remove him and found that during that visit that I had a PO Box.
After coming back from a counseling appointment with Dee Collins, I noticed that he opened and went through the safe box inside my drawer containing the passports, birth certificates, Chrysler title, and other important small papers. As far as I could tell, nothing was removed but I could not find the life insurance policies and copies of the wills that were under that same box.
On December 4, 2009, while I was going through the safe box again to make sure that nothing was taken, I found the life insurance policies misplaced in my drawer and my daughter Daphnee confirmed that she saw Ken going through that same paperwork the night before while I was at my appointment.
I made the decision to remove the safe box and all important paperwork, including prior years income tax returns and immigration papers out of the house. I put everything in a small suitcase and 2 bags and left it in the trunk of the Camry. I plan on dropping everything off in my Cardinal office before going home.

Cardinal

On December 3, 2009 at 11:11a I received an email from Kathy at Cardinal "Can you call me? I just got a phone call from Ken". I called her immediately and she informed me that Ken asked her to talk to me as a friend, that I was made the biggest mistake of my life. She told me that she could not get involved with it and she hang up.

State Farm (2)

On 12/03/09 Ken called me to tell me that he verified that I had rental insurance through State Farm - he called our agent. I immediately called them and talked to Debbie and asked her not to give any information to Ken about my account (Life, Auto and Rental). She told me that Ken called to try to get information from them but that other than confirming that my rental insurance policy was underwritting, she did not and could not give him any information, including the property address. She told me that she could not be in the middle of our situation since she did not know what was going on.

Chase Leisure Rewards

On December 2, 2009, I logged-in on chase.com and checks the rewards points associated with the Chase Joint Checking Account - 51,440 points were available. I requested (4) $25 Target Gift Cards and (1) $5 Panera Bread Gift Card to be mailed to my PO Box, leaving less than 2,000 points on the account.

Direct Deposit - Payroll

On 12/1/09, I requested that my 12/4/09 payroll check from Cardinal was issue as a live check and provided Vice President with a voided check for my personal Chase Account for the following paychecks.
On 12/2/09, I requested that starting with my 12/15/09 payroll check from McKinney, the direct deposit was to be made on my personal Chase Account. A voided check was provided to the Chairman.

Comcast

Called Comcast on 12/2/09 at 1:10pm to set-up services at the Ashland home. I was unable to do it since the old tenant has not disconnected the services yet. I am to go to the local Comcast office and show the lease, then I should be able to set-up services.

State Farm

Called State Farm on 12/2/09 at 12:15pm, talked to Debbie. Removed the 2009 Toyota auto insurance and the Life Insurance for myself off the automatic withdrawal from our Chase Joint Account which will result to the loss of the multi-vehicles discount (additional $8/month for the 1997 Chrysler and $12/month for the 2009 Camry). Took a rental insurance policy for the Ashland home with $50,000 personal property, $300K liability and $500 deductible, effective 12/24/09 for $106/year (8.83/month). Debbie is to fax over the paperwork so I can return with a voided check to authorized withdrawal from my personal Chase Account.

Nicor Gas

Called Nicor on 12/2/09 at 11:50am to remove my name off the Idlewild home account effective immediately and to set-up new services at the Ashland home starting 12/23/09. Also signed up for Line Guard for $4.95/month. The Idlewild home account has a credit balance of $414. Since the account is in my name, I have the option of having the balance refunded or to leave it, I opted to leave it on the account to offset heating costs.

ComEd

Called ComEd on 12/2/09 at 11:15am to remove my name off the Idlewild home account effective immediately and to set-up services at the Ashland home starting on 12/23/09. The new account will reflect a $195 security deposit, payable in 5 months ($39/month) since it will be the first time that I am a primary customer. The security deposit is to be refunded with interest after 12 months of on-time payments.

Toyota Financial Services

Called Toyota on 12/2/09 at 11:05am to change the contact address to the PO Box. Payoff on the 2009 Camry as of 12/2/09 (valid until 12/14/09) is $20,853.33, last payment was made on 12/1/09 for $540 and next payment due is 1/14/10 for $172.28 (note is $507.58/month)

Disposal Service

Called Disposal Service on 12/2/09 at 11:05am to remove my name off the Idlewild home and to set-up new services on the Ashland home starting 1/6/10 - garbage pick-up is on Wednesdays. I need to provide a container less than 35 gallons for regular waste, the property should have the brown container for recycled waste.

Village - Water Department

Called Village on 12/2/09 at 11:00am to verify that my name is not on the water/sewer account, it is not - no changes were made.

AT&T U-Verse

Called AT&T U-Verse on 12/2/09 at 10:50am to disconnect U-Verse TV U300 ($65/month + $24/month for equipment) and Yahoo Broadband Elite ($35/month) on 1/2/10 - they can only hold a disconnection notice for 30 days. Only remaining on the U-Verse account is Unlimited Voice for 708-xxx-0526 ($30/month)
The U-Verse DVR Receiver and the 2 additional receivers must be returned via UPS before 1/12/10 - go to UPS Store on 183rd street and UPS will package and send to AT&T with the account number and the disconnect number. The final bill will be prorated since we paid one month in advance.

AT&T

Called AT&T on 12/2/09 at 10:45am to disconnect the fax line 708-xxx-9404 effective immediately. The final bill will be sent to the Idlewild address.
